You will input your 5-digit zip code and click the Start button as seen below.,



Next, you will enter data in the below fields. Note: Data must be valid and accurate. If data is invalid, alerts will be populated explaining highlighting data entered incorrectly.,

 - Review

 - Application Completed


Once the application is complete, you will receive an update on your new phone and/or service eligibility.


Additionally, as we have visited properties to launch the new WorkConnect program, we have found that less than 2% of workers did not have a smartphone.
